Welcome back to a new school year!
The FUSE Welfare Fund is committed to continually working to see to the welfare of our members. At our April meeting, the Trustees voted to increase the payout on several benefits. Please refer to the Benefit Fee Schedule to see the new rates paid for Vision, Orthodonture and Crowns as well as the increased amount of implants members may get within their lifetime of coverage. We will be continuing our Financial Counseling Program through Stacy Braun, Inc as well as offering limited Legal Services to members through June 30, 2022. Members are eligible to use the attorney partnering with the Fund to facilitate a Basic Will, Power of Attorney, Living will and or HealthCare Proxy at no cost to them. Please call the Fund office for more details if interested.
SEPTEMBER IS SUPPLEMENTAL COVERAGE ENROLLMENT MONTH
Please take some time to review your Supplemental Coverage needs. As always, members are covered at no cost for all Welfare Fund Benefits. A Benefits at a Glance flyer is attached to this email. A more detailed description of all Welfare Fund Benefits can be found on our website.
If a member elects to add (or remove) a spouse or eligible dependent from coverage, please do so in September. Attached is an enrollment form which needs to be completed and returned with the appropriate documentation (copy of marriage certificate or birth certificates) for any family members you wish to enroll. All required documents must be received by the Fund Manager by September 29, 2021.
If you would like to remove a spouse or dependent from coverage, you must send a request in writing to the Fund Manager. No changes to coverage may be made for any reason without a written request. This includes removing a dependent who has aged out of coverage.
I would also like to remind our members that the Fund no longer carries an Employee Assistance Program, as one is now provided through the CSDNR. Please contact Human Resources for more information.
